Patients browse to the web page and complete the survey you have created. There is a small fee for each survey you upload, but there are no monthly fees.

Download and Analyze

You can log into the provider section of the page at any time and see real time results. You may also download the results at any time for your own analysis. Data can be imported into MS Excel, SPSS or any other statistical software you may use.
